Conflict of Interest Disclosure forms may now be completed, signed and emailed without printing. To sign, type in your name and date. Email the form to the appropriate administrative office in your college by clicking on “Submit Form”. If you prefer, the form can be completed online, printed, signed and sent via campus mail to the appropriate office for review.
The information provided on the disclosure is confidential and will be used only as necessary to report, review, and manage or eliminate conflicts of interest.

Under this policy each college establishes procedures for filing disclosures, administrative review of the disclosures, and management of conflicts. Information on college procedures can be obtained by contacting the appropriate college dean's office.
Sample Management Plan
Because every COI situation is different, it is not possible to provide a universal management plan. Department heads, departmental or college COI committees or others responsible for COI management should use this document to assist in the development of Conflict of Interest Management Plans that adequately address the issues specific to each instance of COI.
